Parental Consent for Counseling Form

In order for minors to receive ongoing counseling, written parental consent is required. Students who are under 18 years of age must have their parents sign, date, and return a Parental Consent for Counseling Form before scheduling an initial assessment unless they are in crisis. If students under age 18 are in crisis they should call or walk into the Counseling Center and identify the need for a crisis session.

Please send completed forms to the Counseling Center with the student, fax it to 407.823.5415, or mail it to:
